I've seen this info posted in several places, but not at action-inside, so maybe someone here will find it useful.
Since BF2 doesn't delete old cache files when you change your settings, new cache files get mixed with the old ones and the game apparently uses that big mess.
Follow these steps every time you change the game's settings:
1) Start BF2
2) Change the settings
3) Exit BF2
4) Navigate to My Documents\Battlefield 2\mods\bf2\cache
5) Delete all files beneath the cache folder (some may be hidden, and some may be in folders beneath cache)
6) Start BF2, join a game, wait for shaders to optimize, and have fun! It will load slow the first time, but then you will get higher fps and less stuttering
